- Appointment times are reserved specifically for you.
- Please provide us with 48 business hours notice if an appointment needs to be changed.
- Changes made within 48 business hours of your appointment will result in a broken appointment fee.
- Missing appointments voids any warranty work and whitening benefits.
Warranty Agreement
- Our warranty provides protection for all work completed in our office.
- Fillings carry a five year warranty, crowns and bridges carry a lifetime warranty.
- Our warranty does not cover recurrent decay.
- The warranty becomes voided when re-care visits are missed.
